Your arrangement is similar to what we do at Queen's in Kingston, Ontario.
I thought that this policy was standard in North America at least?
Could the forms signed be held in the School of Grad Studies and
therefore not come to the attention of Librarians? We arrange for
the binding and microfilming (by UMI) of theses, so we see the
letters signed by students as a matter of course. Occasionally,
a thesis will be protected owing to pending publication, but this
happens in a minority of cases.
